In-Person SMS Opt-In Process

Enrollment is optional, and staying unenrolled changes nothing else.

Most customers enroll online, but an active Maui Pack and Ship mailbox holder can also choose to enroll in Package Alerts in person, using an optional paper consent record. This page documents that process and shows the exact form we use.

Package Alerts are optional. Enrollment is never required to obtain, rent, or maintain a Maui Pack and Ship mailbox, or to purchase any goods or services. Customers who don't enroll continue to receive our standard notification: a blue package notice card placed inside their mailbox when a package is ready for pickup.

The Process

In-Person Opt-In Workflow

This is the process our retail staff follow when an existing mailbox holder asks about Package Alerts at the counter, at 415 Dairy Road, Suite E, Kahului. The affirmative action of checking the box is what's required to establish SMS consent — SMS consent itself is entirely optional.

    Staff share information

    Staff may provide an existing mailbox customer with information about the optional Package Alerts service.

    Customer reviews the disclosure

    The customer reviews the SMS consent disclosure — message purpose, frequency, data rates, and STOP/HELP instructions.

    Customer may voluntarily check the box

    The customer may voluntarily check the SMS consent checkbox if they wish to enroll in text notifications. The checkbox is never pre-checked.

    Leaving it unchecked changes nothing else

    The customer may leave the checkbox unchecked without affecting their mailbox or any other Maui Pack and Ship service.

    A phone number alone isn't consent

    Providing a mobile number alone does not constitute SMS consent.

    Initials, signature, and date are optional

    If a customer wants to complete the optional written consent record, the initials, signature, and date fields on it are optional and are not required to submit the form.

    Staff record consent accurately

    Staff record the customer as SMS opted-in only when the customer has affirmatively selected the SMS consent checkbox.

    An unchecked box is never treated as consent

    An unchecked checkbox must never be treated as SMS consent, regardless of any other information the customer has provided.

The actual, unmodified blank form a customer may complete at the counter if they choose to. It mirrors the disclosures and consent language on our online enrollment page, and makes clear that the SMS checkbox, initials, signature, and date are all optional.

Completing this form — including the SMS checkbox, initials, signature, and date — is entirely optional. A customer can decline any or all of it without affecting their mailbox or any other Maui Pack and Ship service.

Separate From Mailbox Rental

Package Alerts Is Not How You Get a Mailbox

Package Alerts is an optional SMS add-on available to customers who already hold an active Maui Pack and Ship mailbox. It is not part of, and is never required for, mailbox rental. Obtaining a mailbox is a separate in-person process that includes USPS Form 1583 and identity/address verification, completed independently of any Package Alerts enrollment choice.
